<div> <b>Job Summary:</b></div> <p>Kaiser Permanente (KP) nurses advance the art and science of nursing through clinical and service excellence, professionalism, and leadership. The Licensed Practical Nurse - Virtual Care provides virtual nursing care to patients, members, and families to promote health and wellness and prevent illness. Collaborates with clinicians, leaders, and members of the virtual care team to deliver high quality care in accordance with scope of practice. Assist with patient education. Documents clinical information in the medical record.</p><div><br><b>Essential Responsibilities:</b></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"> <li>Monitors electronic messaging related to patient and provider requests and inquires within scope of practice and as delegated by RN. Manages messages in a timely and effective manner to resolve patient requests and concerns.</li> <li>Determines appropriateness and acuity of needs based upon electronic messaging content and initiates follow-up telephone contact within scope of practice.</li> <li>Directs messages and transfers calls if outside of scope of practice to RN for management and resolution.</li> <li>Determines patient and family health education needs, provides health teaching and counseling and evaluates effectiveness of the education process.</li> <li>Accurately documents patient data according to established guidelines and policies.</li> <li>Participates in quality improvement activities or initiatives.</li> <li>Serves as an advocate for the patient.</li> <li>Completes other department-specific procedures and nursing care within scope of practice and skill.</li> <li>Adheres to KP compliance standards and Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licensure requirements (if applicable), and Kaiser Permanentes policies and procedures.</li> <li>May perform other duties as assigned.</li> </ul><br><br><div> <b>Basic Qualifications:</b></div> <div> <u>Experience</u></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"> <li>N/A.<strong> </strong></li> </ul><div> <u>Education</u></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"><li>Graduate of an accredited nursing (LPN/LVN) program.</li></ul><div> <u>License, Certification, Registration</u></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"><li>Practical Nurse License (Georgia) required at hire<br></li></ul><div> <b>Additional Requirements:</b></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"><li>Complete a Customer Service Assessment.</li></ul><div> <b>Preferred Qualifications:</b></div> <ul style="margin-top: 0;margin-left: 1px;margin-bottom: 0;"><li>Healthcare experience preferred.</li><li>Customer service aptitude demonstrated through Customer Service Assessment.</li></ul>
