Position Overview

We’re seeking compassionate and experienced Nurse Practitioners (NPs) to provide remote, after-hours care through our telehealth platform. This is an excellent opportunity for providers looking to supplement their current work, work from home, or contribute to a healthcare mission that values access, equity, and provider autonomy.

We partner with Federally Qualified Health Centers (FQHCs) and other Community Health Centers (CHCs) to extend care into the evenings and weekends, ensuring patients receive timely, high-quality care from trusted professionals.

Key Responsibilities

Provide telehealth consultations during after-hours shifts (evenings, weekends, holidays)

Discussing and reviewing patients' medical history, symptoms, allergies, and current medications.

Asking patients situation-specific questions to formulate accurate diagnoses.

Formulating suitable treatment plans to address non-threatening conditions, such as common colds, cases of flu, ear infections, urinary tract infections, allergies, and rashes.

Advising patients on suitable diets and exercise regimens to improve their overall health.

Maintaining accurate records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.

Collaborate with our physician leads for escalation or backup, as needed

Follow up on labs, referrals, and patient questions as appropriate

Ideal Candidate

An active and unrestricted national certification from one of the following organizations: American Academy of Nurse Practitioners; American Nurses Credentialing Center

Minimum 2 years of clinical experience

Confident working independently and making decisions remotely

Comfortable with technology and virtual care platforms

Passionate about health equity and closing access gaps

No barriers to obtaining credentialing and malpractice insurance.

Bilingual (Spanish or other languages) is a plus but not required
