Registered Nurse (RN) – Health Coach Consultant (Remote) Must reside in EST.

Position Overview

We are seeking a highly motivated and experienced Registered Nurse (RN) with a strong background in chronic condition education and telephonic 1:1 member coaching. This is a remote, full-time position (40 hours/week) providing health coaching, assessment, and care support to members with chronic health needs. The RN Health Coach Consultant plays a key role in helping members understand their conditions, adopt healthier behaviors, and achieve optimal health outcomes through a collaborative approach. Applicants must be able to work independently in a remote environment while providing exceptional member support, education, and clinical insight.

Work Schedule

• Monday–Friday: Standard business hours supporting EST (8:00–4:30 PM or 8:30–5:00 PM), including two evening shifts per week (11:30 AM–8:00 PM EST) • Occasional Saturday support may be required based on business needs • One late-Friday rotation every 4–6 weeks • Remote Work

Primary Responsibilities

Member Assessment & Engagement: • Conduct clinical assessments and review health data to evaluate member needs and program eligibility • Assess member readiness, health literacy, and technology capability • Engage members in chronic condition support and personalized coaching   Health Coaching & Lifestyle Modification: • Provide individualized telephonic health coaching using motivational interviewing • Support sustainable behavior change and promote positive health outcomes • Empower members to make informed healthcare decisions   Clinical Quality & Care Coordination: • Apply clinical guidelines, policy criteria, and regulatory standards • Consult with supervisors and Medical Directors to address barriers • Identify and escalate quality-of-care concerns • Provide accurate health information to support understanding   Monitoring, Evaluation & Documentation: • Develop and maintain individualized care plans • Document all interactions per regulatory and accreditation requirements • Monitor progress and adjust plans to meet member needs

Required Qualifications

• Registered Nurse must hold an unrestricted nursing license and reside in a compact state with multi-state/compact privileges and have the ability to be licensed in all non-compact states and must reside in the Eastern Time Zone. • 3+ years of acute adult Medical-Surgical RN experience • High-speed, hard-wired internet connection • Proficiency with Microsoft Office (Word, Outlook, Excel, Teams, PowerPoint), multi- system navigation and keyboarding • EMR documentation experience • Excellent communication skills • Ability to work independently and collaboratively

Preferred Qualifications

• Telephonic chronic condition coaching experience • Managed care experience • EPIC EMR familiarity

Education

• Associate Degree in Nursing (ADN) required • Bachelor of Science in Nursing (BSN) preferred       Anticipated Weekly Hours 40    Time Type Full time    Pay Range The typical pay range for this role is:   $54,095.00 - $116,760.00  This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
