Position Summary:

We are seeking an experienced, detail-oriented Home Health Intake Coordinator to join our growing team. The Intake Coordinator plays a crucial role in ensuring smooth and timely patient admissions by gathering and verifying all necessary information and coordinating with physicians, referral sources, and field staff. While an LVN license is preferred, it is not required. However, prior experience in home health intake or admissions is essential.

Key Responsibilities:

Receive and process referrals from hospitals, physicians, and other healthcare providers.

Obtain and verify all necessary patient information, including demographics, insurance coverage, and clinical documentation.

Ensure timely data entry of referral and admission details into the electronic medical records (EMR) system.

Qualifications

Required:

High school diploma or equivalent

Minimum 1–2 years of experience

Proficient in computers, including Microsoft Excel

Ability to learn EMR systems

Bilingual: English & Spanish (spoken and written)

Strong communication and organizational skills

Some knowledge of Medicare, Medicaid, and private insurance guidelines

Preferred:

LVN license

Associate’s or bachelor’s degree in a healthcare-related field
